I avoid upright rows, behind the neck presses and behind the neck pull downs.
I also haven't done parallel bar dips for a while because they were hurting my shoulder. But I'm pretty sure I intially recieved the shoulder injury from upright rows.

^What Bigjosh listed are the only ones that come to mind for me. Behind the neck everything is going to depend on the individual. Dips are great but I feel like once you get pretty heavy they get dangerous. For example I was always lean and small so dips have always been easy for me. After I started lifting I ended up being able to do dips with an additional 50lbs with a belt. However I felt them tremendously on my shoulders and the further down I went the more discomfort I would get.

Took them out completely and haven't done them for about 6 months. Might try them again however and start with lighter weight soon.

There's not that many "bad" exercises. I think there's a lot of exercises however that aren't needed or dont yield much benefit, most of them being machines.

Definately behind the neck presses like the others said. For me personally flat db press kills my rotator cufs, and I would throw in any excercise with a smith machine.

Only you know the answer. People say upright rows. They give me no issues. Wide-grip bench press is bad for my shoulders, but I've seen many in the gym perform them with no issues.

Everybody is different. Try an excercise if you want to add it to your workout. Go light initially and find out if it will give you issues. If it does, skip it; otherwise use it freely.

Great observation. I answered for anyone, long arms or short arms. Like you said, it doesn't make a difference.

I don't like behind the neck presses, but upright rows have been critical in my shoulder development. I usually do them with dummbells as opposed to a barbell though which helps me have a more natural ROM.

I do nothing behind my neck (OH press, lat pulldown, etc.) because it is bad for your rotators.
